Endovascular aneurysm repair (EVAR)

Endovascular aneurysm repair (EVAR) is a minimally invasive procedure used to treat abdominal aortic aneurysms (AAAs) and thoracic aortic aneurysms (TAAs). It is an alternative to open surgical repair and offers several advantages, including shorter hospital stays, faster recovery times, and reduced risk of certain complications.
Here's how the EVAR procedure is typically performed:
  • Pre-procedure Preparation: Before the procedure, the patient undergoes imaging tests, such as CT or MRI angiography, to assess the size, location, and suitability of the aneurysm for endovascular repair. This helps the medical team plan the procedure.
  • Anesthesia: EVAR is performed under general anesthesia or sometimes with local anesthesia and sedation to ensure the patient is comfortable and unconscious during the procedure.
  • Access: The procedure is usually initiated through small incisions in the groin, where catheters are inserted into the femoral arteries or iliac arteries.
  • Guidewire Insertion: A guidewire is threaded through the catheter and guided to the site of the aneurysm within the aorta.
  • Stent-Graft Placement: The stent-graft, which is compressed onto a delivery system, is advanced over the guidewire and positioned at the site of the aneurysm. Once correctly placed, the stent-graft is expanded, and the fabric covers the weakened area of the aorta.
  • Deployment and Seal: The stent-graft is deployed, and its ends form a seal above and below the aneurysm, creating a new pathway for blood flow and excluding the aneurysm from the circulation.
